The superficial fat pad is removed first, then the platysmal muscle is opened to address the subplatysmal fat. The subplatysmal fat is removed to be flush to the anterior belly of the digastric or submental gland. Once the desired amount of fat is removed, the platysmal muscle is tightened, and the skin is approximated and closed with a plastic surgery closure technique.
