Dr. Totonchi graduated at the top of his class from the Tabriz University of Medical Sciences. Since 2001, he has been a part of North East Ohio’s medical community, completing his plastic surgery residency in University Hospital, and then continuing with fellowships in facial cosmetic and pediatric craniofacial surgeries.
Dr. Totonchi joined MetroHealth Medical Center in 2010 as a Professor at Case Western Reserve University’s School of Medicine. He serves as onsite Director of Plastic Surgery Residents at MetroHealth Hospital, where he is also the Medical Director of the multidisciplinary Craniofacial Disorders Clinic (CFD). There, he leads a highly trained team of professionals, caring for children with facial disorders, such as cleft lip, cleft palate, and facial deformities.
He performs a wide variety of cosmetic and reconstructive procedures with a focus on facial rejuvenation, including:

- Rhinoplasty
- Facial Masculinization Surgery
- Facial Feminization Surgery
- Non-Surgical Enhancements
In addition, he is also one the few plastic surgeons in the country who has trained under Dr. Bahman Guyuron to perform surgery and help patients with severe migraine headaches. Dr. Guyuron’s innovative techniques are at the forefront of advancements in surgical treatment options for migraine headaches. Dr. Totonchi is proud to be able to offer migraine surgery as an option for his patients who suffer from migraine headaches.
In 2023 Dr. Totonchi became the President of the Migraine Surgery Society, an esteemed think-tank comprising leading professionals who actively define and advance the realm of migraine surgery.
