Secondary Rhinoplasty

Rhinoplasty refers to plastic surgery of the nose.  A secondary rhinoplasty is a corrective nasal surgery that changes the shape or function of the nose after a primary rhinoplasty.  A secondary rhinoplasty is also known as a “revision rhinoplasty”.  A secondary rhinoplasty or a “revision rhinoplasty” is indicated for continued aesthetic or functional concerns that were not properly treated with the primary rhinoplasty, or the outcome was not aesthetically pleasing to the patient.  

Patients look to address persistent imperfections, newly created concerns, or worsening issue after their primary rhinoplasty.  A secondary rhinoplasty is a much more complicated procedure than the primary rhinoplasty due to scar tissue, limited cartilage remaining for repairs after a primary rhinoplasty, and an altered nasal structure.

Common concerns which prompt secondary rhinoplasty consultations:

*nasal airway obstruction    *nasal bone or cartilage collapse

*nasal valve collapse             *asymmetry

*irregular shape                      *excessive/minimal nasal tip projection

*scar tissue                              *excessive nasal tip narrowing or “pinched tip”

A secondary rhinoplasty can be done via the “closed” or “open” approach, depending upon the amount of structural revisions necessary.  Major structural changes require an “open” rhinoplasty approach.  An “open” rhinoplasty requires an additional incision between the two nostrils, or the columella, so the skin can be lifted to view the underlying structures.  A “closed” rhinoplasty only requires an internal nasal incision to access the nasal structures.  May times, in a secondary rhinoplasty, there is a need for cartilage grafts to shape and support the nasal bones.  The cartilage is generally obtained from the ear or ribs for grafting. The primary rhinoplasty usually depletes the amount of nasal cartilage available for grafting thus needing to use the ear or rib cartilage for the secondary rhinoplasty repair.
